This was also a personal triumph for Napoli coach Luciano Spalletti, who was badly treated in his last spell at Roma and was his first victory against his adversary and Roma coach Jose Mourinho.
Osimhen fired home from a tight angle and despite the tight attention of Roma defender Chris Smalling to give Napoli all three points in Rome.
He made up for a miss moments earlier in this bruising clash.
He has now netted four goals in Serie A and a goal in the UEFA Champions League.
Napoli are now three points clear at the top of the table with 29 points from 11 matches.
